SUPPORTIVE HOUSING PROGRAM QUALITY ASSURANCE & IMPROVEMENT

Staff conduct systemwide quality assurance and improvement for a nine-agency, fully integrated collaborative program that provides transitional housing, clinical services, social and vocational rehabilitation to individuals who have been homeless and have a serious mental illness and co-occurring substance abuse disorder from New Haven, Hamden, Bethany, and Woodbridge. The collaborating agencies include the Connecticut Mental Health Center, lead agency; ALSO-Cornerstone, Inc.; Columbus House, Inc.; Continuum of Care, Inc.; APT Foundation; Easter Seals Goodwill Rehabilitation Industries; Fellowship, Inc.; Marrakech; and Yale Behavioral Health.
